What to remember when you come for your HGV medical or taxi medical ...

We need to know if you have particularly strong glasses. If you have very thick glasses, or you're not sure, you must bring your prescription with you. If they are thicker than +8 strength we need to know. You can read more about this on our FAQs page.

Make sure you have your medical form for the doctor to fill in. Please fill in the section with your name and address and GP details before you arrive. (If you have a DVLA D4 medical form this is on the front page.)

Bring identification with you, such as a passport or photo driving licence. If you don't have either please call us.
